62 year old woman with history of high cholesterol and smoking admitted with severe pain in legs.  Unable to walk.  No palpable pulses in legs.  Angiogram shows total occlusion of abdominal aorta just below the renal arteries.

Aorta reconstitutes via collaterals just above the aorto-iliac bifurcation.  Iliac arteries appear very small as they are "under-filled".  Blood pressure in iliac arteries only 40 mmHg!

Aortic occlusion crossed with a wire and then treated with balloon angioplasty which restored some flow.  Intravascular ultrasound used to measure exact diameter of aorta and length of occlusion in preparation for placement of Viabahn BX covered stent...

Excellent final result after placement of 11x59 mm Viabahn BX covered stent.  Total procedure time approximately 45 minutes!  Normal blood flow to legs restored without surgery!
